Holding her signature mask with that mischievous, ice-melting grin, Oriha’s expression is pure dynamite. Her bright, glistening eyes and the way she’s angled toward the viewer create an instant connection, as if she’s daring you to step into her world. It’s not just a pose—it’s a trap, and I’m happy to fall for it every time.
Beyond Oriha herself, the color scheme is absolute perfection. The bold red-and-black background cranks up the intensity, making her pop off the cover in a way that’s impossible to ignore. She’s the most striking character in the series, hands down. Tell me I’m wrong. You can’t. That’s exactly why Volume 9 stands out. Every cover featuring Hitsugi carries an undeniable presence—bold, powerful, and packed with personality—and this one is no exception.
Here, we see Hitsugi mid-battle, her war-torn outfit barely hanging on, shield raised, all while casually talking her ish like the absolute legend she is. But what really seals the deal is the minimalist red-and-white background, which keeps the focus exactly where it should be—on her. It’s simple. It’s striking. And, let’s be honest—it’s hot.

Kaoru and Kaori are superpowered twins. Their shadowy past merges with Hitsugi’s own. But she’s breaking free of her past and committing herself to doing Black Label’s good work. The first order of business: wiping out a pair of red-haired blemishes on the face of the city!
6. Volume 15: Creative Concept, New Color Combos and Contrasting Personalities
Volume 15 delivers one of the hottest and most badass characters in Triage X—Goryu. She’s mid-attack, hurling ninja stars with precision, and her pose? Absolutely mesmerizing. But what really steals the spotlight is her face—intense, fierce, and dripping with raw emotion. And then… there’s Kyouji Tobishiro. Pathetic. The dude looks like he’s about to pee himself, and honestly? It’s hilarious. But what truly elevates this cover is the chaotic, black-heavy background. It’s messy, erratic, almost overwhelming—yet it works perfectly, balancing the color scheme while amplifying Goryu’s dominance. This cover isn’t just hot—it’s straight-up electrifying.

5. Volume 24: Back to Back.
Volume 24 of Triage X brings something fresh and dynamic to the table, featuring Kaoru Murasaki and Sayo Hitsugi in a composition that just works. Hitsugi, towering above with dual pistols in hand, faces away with a calm, collected demeanor, while Kaoru lunges forward, charging toward the reader. It’s a setup we haven’t quite seen before, and that uniqueness alone makes it a standout in the series. There’s something that just pops, and that quality makes it somewhat unique.
What also sets this cover apart is its unexpected color palette. Instead of the usual high-contrast chaos, we get a soft yet striking mix of powder blue, white, and black—a rare sight in this action-packed series. Sure, I’ll admit it has a bit of a chaotic feel, making it harder to digest at first glance, but that organized chaos is precisely what makes it memorable among the other Triage X manga covers.

As the battle rages on, the Black Label forces are divided further when Oriha is caught in a Syringe trap. Cruelly manipulating Oriha by exploiting her greatest vulnerability, Kaname seems sure she’s bested her rival hacker at last. Will Oriha break free, even when giving in to Syringe means reuniting with someone dearly precious to her?
4. Volume 22: Oriha Brings the J-Idol Energy, With Stars In Her Eyes and Gear to Remember
Volume 22 is pure, unfiltered sugar rush—a cotton-candy explosion of fun and energy, starring none other than our favorite idol, Oriha Nashida. She’s rocking a wild mix of cute and badass, wielding a modified bat, sporting sleek shin guards, and topping it off with her signature brown coat. And let’s not forget the princess dress—because why not add a little extra flair? Of course, those signature starry eyes are still shining, giving her that irresistible charm.
While her expression could’ve had a bit more punch, the awkward yet dynamic pose just works, and the simple background? It’s genius. With all the colors popping just right, Oriha takes center stage like the star she is. Playful, bold, and undeniably eye-catching—this cover is a total show-stealer.
Triage X, Vol. 22
Black Label rushes to Mochizuki’s side, their progress inhibited by the last of Syringe’s goons. In their absence, Reika takes a moment to relish her victory and the revelation of Fiona’s true nature. Fiona and Mochizuki represent the key to unlocking D99-and unleashing a whole wave of terror that will spread to the whole world!
3. Volume 25: Elegance and Power, Abstract In Nature
Volume 25 of Triage X is elegance with an edge, featuring Reika Togo in a striking outfit that oozes sophistication and mystery. With one hand resting on her head and her carbon fiber-like leg tearing through her pants, this cover is a masterclass in contrast and creativity. The level of detail on her cybernetic limb is next-level, making it one of the most visually captivating elements of the entire series.
What makes this cover truly pop is its bold black-and-white background, which seamlessly blends into Reika’s white coat and black leg, giving it an almost monochrome aesthetic. The only splash of color? That soft lavender-blue hue on her dress, pants, and flowing hair—a subtle yet brilliant touch that makes Reika the undeniable focal point. Reika has sorely underestimated the power of Black Label’s teamwork. Neither Lady Dire’s mighty shield nor the puppet armies of Algol can protect her. At last, the master mind behind humanity’s supposed destruction seems to be up against the ropes! Is Arashi prepared to execute triage?!
2. Volume 26: Splash of Color, Great Poses, Make for a Manga Cover to Remember
The later volumes of Triage X boast some of the most visually striking manga covers, and Volume 26 is a prime example of that brilliance. With Miki and Yuko front and center, both radiating sheer badassery, this cover delivers a powerful composition. In all honesty, I could swap the 1st place and 2nd place with ease, as they are both among the best Triage X manga covers!

I haven’t made it to Volume 27 yet, but I’m pretty sure this cover once again features Reika Togo—and what a stunning shift in style! Gone are the chaotic backgrounds and action-packed poses that define many of the previous covers. Instead, we get a breathtakingly elegant, nude composition, with a character reaching out towards us, the reader. It’s a moment of pure serenity, intimacy, and intrigue—a cover that pulls you in effortlessly. You can’t help but want to pick up the volume instantly, just to unravel the mystery behind this alluring, captivating presence.
And in all fairness, what truly elevates this cover is the way her flowing hair structures the entire piece. The soft yet striking colors paint the scene beautifully, adding depth and fluidity. It’s artful, mesmerizing, and easily one of the best Triage X manga covers ever.
